Does Your Child Need Speech Therapy?
Therapy Goals is partnering with Solid Foundation Christian Academy to provide on-site speech and language therapy services for students who may benefit from additional support.
A speech delay refers to difficulty producing sounds or speaking clearly, which may affect how well a child is understood by others. A receptive language delay impacts a child’s ability to understand what is said to them, while an expressive language delay affects their ability to use words and sentences to express themselves effectively.
If you would like to request a speech and language evaluation for your child, please complete the attached intake form and return it to the school, or fax it to us at 833-232-1614. You will need a Speech Prescription from your child’s doctor before we can proceed. You may also submit the form online by visiting:
www.therapygoalsllc.com/appointments.
We accept most major insurance plans. To begin the evaluation process, we will also need:
These documents may be faxed to 833-232-1614 as well.
